  • Facilitate patient access to medical and specialty providers, educate patients on community resources, and keep detailed up-to-date documentation.
Requirements:
  • 2 years of experience in social services, with an Associate's degree required.
  • Bilingual Spanish is strongly preferred.
Estimated Salary: $55,000 - $65,000 per year
