We are seeking a skilled and compassionate Patient Care Manager to join our team at Assured Home Health & Hospice in Olympia, WA. As a Patient Care Manager, you will play a vital role in providing quality support to our patient-facing teams and ensuring the best possible outcomes for our patients.

About the Role

The Patient Care Manager will be responsible for supervising and coordinating clinical services, providing and directing provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

Key Responsibilities
  • Supervise and coordinate clinical services to ensure high-quality patient care
  • Provide and direct provisions of nursing care to patients in their homes
  • Ensure compliance with applicable laws, regulations, and agency policies
  • Collaborate with other members of the healthcare team to achieve optimal patient outcomes
  • Coordinate determination of patient home health benefits and medical necessity
Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
