EMERGENCY ROOM LIAISON RN - CASE MANAGEMENT


Job Overview:

The Care Manager will operate in dual environments on a rotating schedule, orchestrating patient discharges and follow-up care for individuals enrolled in the health plan who are admitted to a partner facility, while managing a caseload within one of the health plan's medical centers. Flexibility in scheduling is required, including some weekends and holidays. The primary focus is to ensure optimal health outcomes for patients while effectively utilizing healthcare resources. Collaborating closely with primary and specialty care teams, the Liaison Nurse will develop a comprehensive care plan that promotes adherence to medical directives, enhances functional status, and empowers patients in self-management. This role serves as a vital link across the care continuum and between the health plan and external providers, offering insights to leadership regarding service quality and care delivery.


Key Responsibilities:
  • Evaluate patients referred for case management to confirm they meet established criteria. Assess all aspects of the patient's status—physical, mental, financial, and psychosocial—using standardized criteria to identify needs. Create a patient-centered case management plan that includes goals, objectives, and timelines. Continuously assess the capability of designated caregivers to support the patient. Implement interventions based on evidence-based guidelines and maintain ongoing communication with patients and their families.
  • Collaborate with healthcare professionals to refine the care plan, addressing any variances and monitoring patient responses to interventions. Document all interactions and interventions in accordance with policy and procedure. Participate in evaluating the effectiveness of the case management program.
  • Guide the care plan to ensure a seamless continuum of care. Facilitate referrals for home health, long-term care, hospice, and other necessary services. Collect data to assess the impact of care coordination on quality outcomes, patient satisfaction, and system improvements. Assist patients in understanding their treatment plans and promote education through referrals to relevant resources.
Qualifications: Experience
  • Minimum of three (3) years of recent clinical experience in medical/surgical or ambulatory settings required.
Education
  • Bachelor's degree or Bachelor of Science in Nursing.
License, Certification, Registration
  • Registered Nurse License (Washington) required at hire
  • Basic Life Support certification required at hire
  • Case Manager Certification within 24 months of hire
Additional Requirements:
  • Demonstrated independent nursing judgment and skills, utilizing evidence-based clinical decision-making.
  • Knowledge of chronic disease management and collaborative care planning.
  • Proven ability to effectively collaborate with care team members.
Preferred Qualifications:
  • At least two (2) years of RN experience in utilization review, ambulatory case management, or care coordination.
  • Bachelor of Science in Nursing preferred.
