We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Assured Home Health & Hospice. As a Patient Care Manager, you will play a vital role in ensuring the delivery of high-quality patient care and services to our patients in their homes.

Key Responsibilities:
  • Supervise and coordinate clinical services to ensure the delivery of quality patient care
  • Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs
  • Review assessments and plans of care daily and consult with clinicians as necessary
  • Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals
Requirements:
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
