Job Summary

We are seeking a highly skilled and compassionate Nurse Care Manager to join our team at Comprehensive Community Action. As a key member of our healthcare team, you will play a vital role in providing comprehensive care coordination services to patients with chronic health conditions.

Key Responsibilities
  • Conduct thorough patient assessments, including medical, psychosocial, and functional evaluations.
  • Develop and implement personalized care plans, focusing on disease education, self-management support, and preventive screenings.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families, promoting healthy behaviors and disease management.
  • Monitor patient progress, adjust care plans as needed, and ensure timely follow-up.
Requirements
  • Current licensure as a Registered Nurse (RN) from an accredited school.
  • 3-5 years of experience in community health, public health, or chronic disease management.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
