Senior Care Physician

We utilize a collaborative Interdisciplinary Team (IDT) approach to assess and guide participant care, which is fundamental to our values, culture, and mission.
The Senior Care Physician is responsible for conducting thorough in-person evaluations and collaborating with the IDT to formulate personalized care plans for each participant. Under the guidance of the Medical Director, the Senior Care Physician oversees the comprehensive care of participants in various settings, including nursing homes and hospitals, ensuring seamless admissions and discharges for both acute and long-term care. This role also involves providing leadership and support to Nurse Practitioners and other IDT members.
Key Responsibilities:
- Conducts assessments, develops care plans, and provides ongoing medical support to participants.
- Collaborates with community physicians and the IDT for effective discharge planning from acute or long-term care facilities.
- Actively engages in IDT meetings, sharing updates on participant conditions and contributing to care planning and coordination of 24-hour care delivery.
- Maintains regular visits to nursing home participants based on their individual needs.
- Participates in a rotating schedule for night and weekend on-call duties.
Qualifications:
- Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Doctorate Degree in Medicine.
- Board Certification in Internal Medicine, Family Practice, or Geriatrics is preferred.
- Active California Physician License without restrictions.
- Current state CDS Certification and DEA registration.
- Eligibility for enrollment to provide services to Medicare and Medi-Cal participants.

Salary range for this position is $250,000 - $280,000 per year, plus bonuses and equity. WelbeHealth offers a competitive total rewards package, including 401k matching, healthcare coverage, and a variety of additional benefits. Actual compensation will be adjusted based on experience and qualifications.
