Our mission is to provide a comprehensive suite of medical services that address the complete well-being of our participants, emphasizing holistic care rather than merely treating symptoms. We pride ourselves on a collaborative Interdisciplinary Team (IDT) approach, which is essential to our core values and organizational culture.
The Senior Care Physician is responsible for conducting thorough in-person evaluations and collaborating with the IDT to formulate individualized care plans for each participant. Under the guidance of the Medical Director, this role encompasses overseeing participant care in various settings, including nursing facilities and hospitals, ensuring seamless admissions and discharges for both acute and long-term care, and coordinating with medical specialists to facilitate effective care management. Additionally, the Senior Care Physician provides mentorship and support to Nurse Practitioners and other IDT members.
Key Responsibilities:
- Conducts assessments, develops care plans, and provides ongoing medical support to participants.
- Collaborates with community physicians and the IDT to ensure effective discharge planning from acute or long-term care environments.
- Actively engages in IDT meetings, sharing updates on participant status and contributing to care planning and 24-hour care coordination.
- Maintains regular visits to nursing homes to oversee participant care based on individual needs.
- Participates in a rotating schedule for night and weekend on-call duties.
Qualifications:
- Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Doctorate Degree in Medicine.
- Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred).
- Valid California Physician License with no restrictions.
- Current state CDS Certification and DEA registration.
- Eligibility for enrollment to provide services to Medicare and Medi-Cal participants.
