YOUR CONTRIBUTION TO OUR SUCCESS

A Lead RN Medical Care Manager plays a critical role in our organization by utilizing their clinical expertise to oversee the Ryan White Medical Case Management program and Standard of Care.

The successful candidate will work closely with our providers and Healthcare Center team to ensure that Care Management services are delivered effectively to the Ryan White eligible population.

Key responsibilities include:


Providing training and clinical guidance to the team


Monitoring services entered into the Care Management Information system


Collaborating with assigned supervisors to grow EMA/TGA


Contributing to the measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services


Participating in external support groups, consortiums, etc.

Requirements:

Bachelor's degree in Nursing (BSN) or License Practical Nurse (LPN) with at least 3 years of Case Management experience.

Ability to travel in your region up to 50% of the time.
