Job Title: Care Management Team Lead

Job Summary:

The Care Management Team Lead will oversee and direct all levels of care management team members, including RN Care Managers, Lead Care Management RN, Care Management Coordinators, Discharge Planners, and Care Coordinators. This role will also supervise and cover supervision for other management in the Care Management Department.

Key Responsibilities:

  1. Supervise and direct care management team members to ensure the delivery of high-quality care management services.
  2. Collaborate with the Director of Care Management to develop and implement care management policies and procedures.
  3. Provide clinical supervision to care management team members, including coaching, corrective action, and educational needs.
  4. Oversee the electronic referral system, documentation of care management through Cerner Healthcare EMR, and handoff processes.
  5. Work closely with ED Care Coordinators, ED Care Managers, Social Work, and Transitional Care Navigator staff to ensure seamless patient care transitions.
  6. Establish and monitor management metrics, including quality, outcomes, productivity, and financial performance.
  7. Design and coordinate formal and informal training for staff and providers regarding post-acute services and care management services.

Requirements:

  • Bachelor's degree in nursing required; Master's degree in a related area preferred.
  • Five years of acute care hospital, health plan, and/or ancillary services experience required.
  • Prior supervisory or management experience in a clinical role required.
  • RN license required; CCM or ACM certification required within 2 years of hire.
