Key Responsibilities:

  • Provide clinical guidance and oversight to care management services
  • Collaborate with healthcare providers and team members to ensure high-quality care
  • Monitor and evaluate care management services to ensure compliance with regulatory requirements
  • Develop and implement training programs for care management staff
  • Conduct regular meetings with team members to discuss challenges and barriers

Requirements:

  • Bachelor's degree in Nursing (BSN) or License Practical Nurse (LPN) with at least 3 years of case management experience
  • Ability to travel up to 50% of the time
  • Strong clinical knowledge and experience in care management
