Private Duty Nurse RN-Part Time Days

About the RoleAs a Private Duty Nurse RN, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our care team to develop and implement personalized care plans that meet their unique needs.
Responsibilities- Implement and coordinate care plans in accordance with physician orders
- Conduct thorough patient assessments and develop individualized care plans
- Administer medications, treatments, and therapies as prescribed
- Document all care activities and patient progress in a timely and accurate manner
- Provide education and support to patients and their families on disease management, self-care, and prevention strategies
- Collaborate with the care team to ensure seamless care coordination and communication
- Current, unrestricted RN license in the state of practice
- Current CPR certification
- 6 months of recent experience as a licensed nurse in a clinical care setting
- Home health experience preferred
