Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of patients' treatment plans that support their strengths, abilities, needs, and preferences, facilitating their involvement throughout the rehabilitation process.
- Document findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of patients, families, and caregivers.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Currently licensed as an (RN) Registered Nurse in the state(s) of practice according to law and regulation.
- Associates degree required, BSN preferred.
- 2+ years experience in case management or 3+ years experience in clinical nursing (i.e., acute care area: ICU, PCU, Med Surg-Tele, Oncology) required.
- InterQual experience preferred.
- Certification in Case Management preferred.
