RN Case Manager

We are seeking a highly skilled RN Case Manager to join our team at HCA Florida Mercy Hospital. As a RN Case Manager, you will play a critical role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, and caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Currently licensed as an (RN) Registered Nurse in the state(s) of practice according to law and regulation.
- Associates degree required, BSN preferred.
- 2+ years experience in case management OR 3+ years experience in clinical nursing (i.e. acute care area: ICU, PCU, Med Surg- Tele, Oncology) Required.
- InterQual experience Preferred.
- Certification in Case Management preferred.
HCA Florida Mercy Hospital is a 488-bed facility, accredited by the Joint Commission, and offering a wide range of medical specialties, including emergency care, heart, and vascular care, and maternity and neonatal care. We are a Level II NICU hospital and CARF accredited for inpatient rehabilitation.
