Youth Support Case Manager

This program emphasizes family-centered support in either home or community environments, offering a range of services that may include family therapy, therapeutic support, parental guidance, comprehensive assessments, counseling, case coordination, and crisis intervention tailored to the unique needs of each youth and family. The intensity and type of services provided are customized to adapt to the evolving requirements of the youth and family throughout the treatment process.
Each component of our services can function independently, as part of a cohesive NFI team, or be seamlessly integrated into broader treatment teams that may involve hospitals, state agencies, outpatient services, residential treatment facilities, and other community programs.
Key Responsibilities:
- Conduct initial assessments for new program referrals.
- Collaborate with youth, families, caregivers, and other stakeholders to develop a strengths-based, client-centered assessment and treatment plan.
- Deliver individual and family therapeutic support as outlined in the client's service plan.
- Work in partnership with clinicians and other treatment team members.
- Serve as a liaison among the program, families, external service providers, the court system, and referring agencies as specified in the treatment plan.
- Facilitate and/or participate in case conferences and treatment meetings as necessary.
- Advocate for the client's needs within the program and with external organizations.
- Assist in identifying the client's social and recreational needs within their community.
- Support the development of daily living skills, including financial management and utilizing public transportation.
- Help identify the client's post-secondary aspirations, such as vocational training or college readiness.
- Source resources for post-secondary opportunities and foster relationships with potential employers in the community.
- Document necessary progress notes, aftercare plans, service plans, and reviews as required by the program.
- Provide transportation for clients to court, appointments, and home visits as needed.
- Support other staff in crisis prevention and management efforts.
- Perform all duties in compliance with the agency's policies and procedures.
- Bachelor's Degree in a human services field with at least one year of experience working with the target population preferred.
- Associate's Degree in a human services field with one year of experience working with the target population, or a high school diploma or GED with a minimum of two years of experience in a similar role.
- High energy, excellent interpersonal skills, and the ability to thrive in a team environment.
- Strong organizational and communication skills, both verbal and written.
- Flexible availability, including evenings and weekends.
- Valid driver's license is required due to travel obligations.
Full-time employees are eligible for benefits including medical, dental, and vision insurance, a 403(b) retirement plan, three weeks of paid vacation, paid sick leave, and tuition reimbursement up to $2500 per fiscal year.
