About the Role:

As a Hospice RN/ RN Case Manager with Gentiva Hospice, you will have the opportunity to make a meaningful difference in the lives of patients and their families. Our team is dedicated to providing compassionate care and support to those in need, and we are seeking a skilled and empathetic professional to join our team.

Responsibilities:

Provide direct patient care services to hospice patients and supportive care to the patients family
Act as primary liaison between physician, patient/family and hospice team
Empower patients caregivers by educating them on care for their loved one
Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff to provide comfort, peace, and dignity

Requirements:

RN license in the state of requested employment (or eligibility to obtain)
1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
Current drivers license and automobile insurance
Current CPR certification
