Job Title: Director of Case Management

We are seeking a highly skilled and experienced Director of Case Management to lead our team in Fort Pierce. As a key member of our leadership team, you will be responsible for the management of staff and services related to Case Management.

Key Responsibilities:
  • Develop and implement effective Case Management programs to ensure seamless Continuum of Care.
  • Provide education and leadership to staff on clinical care progression, patient education, and resource management.
  • Act as a liaison to the Central Business Office to ensure timely and accurate Accounts and Revenue activity.
  • Maintain 24-hour accountability and ensure compliance with all legal, licensure, and accreditation requirements.
Requirements:
  • Current Registered Nurse (RN) license in the state of practice.
  • Bachelor's Degree in Nursing (BSN) required.
  • Current BLS/ACLS/PALS certification issued by the American Heart Association.
  • UM/CM certification preferred.
  • Minimum of 5 years recent clinical or Case Management experience required.
  • Previous management experience required.
