About this role:

We are seeking a skilled Master Social Worker to join our team at Fresenius Medical Care Holdings, Inc. As a Social Worker with our dialysis clinic, you will play a vital role in providing psychosocial services to our patients.

Key Responsibilities:
  • Assess patients' psychosocial status, strengths, and areas of need to identify effective interventions.
  • Collaborate with the patient and healthcare team to develop and implement care plans that promote positive adjustment, rehabilitation, and improved quality of life.
  • Provide supportive counseling services, patient education, and referrals to community resources to facilitate optimal treatment outcomes.
  • Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referrals and access.
  • Participate in discussions of patient DNR status and ensure patient and/or family understanding of their care.

Requirements:
  • Masters in Social Work
  • State required license
  • 0-2 years' related experience
