Job Summary

Alignment Healthcare is seeking a skilled Advanced Practice Clinician to join our team. As a key member of our care delivery team, you will provide high-quality, patient-centered care to our members in the comfort of their own homes.

Responsibilities
  • Conduct in-home assessments, medication reviews, and health screenings to identify diagnoses and develop personalized care plans.
  • Provide patient education and support to empower members to manage their health and wellness.
  • Collaborate with Regional Medical Officers, care teams, and other stakeholders to ensure seamless care coordination and effective communication.
  • Lead clinical teams, including nurses, health coaches, social workers, and care coordinators, to deliver comprehensive care and support.
Requirements
  • Master's degree from an accredited NP Program or PA program.
  • Preferred one (1) year of prior clinical or home care experience.
  • Active California state Nurse Practitioner or Physician Assistant license.
  • Active RN License and Furnishing number (Must, upon hire) NP only.
  • Active Nurse Practitioner Board Certification/Physician Assistant Certification.
  • NPI Number, DEA, Valid BLS.
  • Valid California driver license and current automobile insurance.
