WORK ENVIRONMENT: Outpatient Care Facility

WORK SCHEDULE:
Monday to Friday, 8:00 AM to 5:00 PM, no weekends or holidays.


This role entails managing phone communications within the practice, assessing patient needs, administering immunizations and medications, providing wound care, preparing patients for consultations, and assisting healthcare providers as required. The candidate will also coordinate outpatient antibiotic treatments with home health services, pharmacies, infusion centers, rehabilitation facilities, and skilled nursing establishments, along with other assigned responsibilities.

The individual will occasionally support other locations as necessary.

ROLE SUMMARY: To deliver nursing services to patients within a healthcare provider's office.

Patient care encompasses individuals across various age groups:

pediatric (infants to 12 years), adolescent (13 to 17 years), adult (18 to 64 years), and geriatric (over 64 years).


KEY RESPONSIBILITIES: Qualified candidates must possess the capability (with or without reasonable accommodation) to perform the following tasks:

Provide clinical guidance via phone or electronic communication based on clinical assessments and triage protocols, adhering to regulatory standards and best practices.

Collaborate with healthcare providers as needed.

Deliver patient-specific education covering preventive care, chronic disease management, acute diagnoses, medications, and other relevant topics impacting patient outcomes.

Prepare patients for appointments using appropriate rooming standards.

Administer injections and medications as directed by the healthcare provider, following departmental protocols and regulatory guidelines.

Assist healthcare providers with various procedures, minor surgeries, and examinations as designated.

Conduct Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.

Collect blood or other laboratory samples and prepare them for further analysis.

Authorize prescription refills as instructed by the healthcare provider.

Assist with tracking tests and monitoring overdue results.

Remove sutures and change dressings on wounds as necessary.

Ensure examination rooms are adequately stocked and sanitized after each patient visit. Maintain and clean medical instruments and equipment post-procedure.

Support healthcare providers and patients in reviewing and updating care plans and assessing patient progress. Follow up with patients between visits as necessary.

Engage in proactive outreach for patients requiring preventive and chronic disease services.

Facilitate care coordination among specialists and transitions of care to gather all necessary patient information.

Assist in coordinating referrals, education, and community resources.

Actively participate in performance improvement initiatives aimed at enhancing the overall patient experience and quality of care.

Participate in daily team meetings with the healthcare provider.

Serve as an advocate for patient experience by maintaining a positive demeanor during interactions with patients, visitors, and colleagues.

Communicate with all patients, visitors, and colleagues in a professional manner, prioritizing customer service.

Handle all patient information confidentially in accordance with privacy policies and procedures.


ADDITIONAL RESPONSIBILITIES:
The following tasks are considered secondary to the primary responsibilities listed above:
Perform various administrative duties as required.

May require some regional travel - a valid driver's license is necessary if traveling.

Participate in on-call responsibilities for this position as needed.

Other duties as assigned.

QUALIFICATIONS

MINIMUM REQUIRED QUALIFICATIONS:
Completion of a Licensed Practical Nurse (LPN) or Registered Nurse (RN) educational program.

Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the relevant state nursing board.

AHA CPR/BLS certification required within 30 days of hire.

This role necessitates occasional regional travel. Ability to travel to multiple sites is required.

PREFERRED QUALIFICATIONS:
One (1) to Two (2) years of nursing experience in a primary or specialty care physician's office.

Prior experience in triage, emergency care, cardiac care, intensive care, or other related nursing specialties.
