POSITION SUMMARY: This role involves delivering nursing services to patients within a healthcare setting. Care is provided across various age groups: pediatric (infants to 12 years), adolescent (13 to 17 years), adult (18 to 64 years), and geriatric (over 64 years).
KEY RESPONSIBILITIES: Qualified candidates must possess the ability (with or without reasonable accommodation) to execute the following tasks:
- Offer clinical guidance via phone or electronic communication based on clinical assessments and triage protocols, adhering to regulatory standards.
- Deliver patient-specific education covering preventive care, chronic disease management, acute diagnoses, medications, and other relevant topics.
- Prepare patients for consultations using established rooming standards.
- Administer injections and medications as prescribed, following departmental protocols and regulatory standards.
- Assist providers with various procedures, minor surgeries, and examinations as directed.
- Conduct Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
- Collect blood or other laboratory specimens for further analysis.
- Authorize prescription refills as per provider instructions.
- Support test tracking and monitor overdue results.
- Remove sutures and change dressings as required.
- Ensure exam rooms are adequately stocked and sanitized after each patient visit. Maintain and clean medical instruments and equipment post-procedure.
- Assist providers and patients in reviewing and adjusting care plans, assessing patient progress, and following up as necessary.
- Engage in proactive outreach for patients needing preventive and chronic disease services.
- Facilitate care coordination among specialists and manage transitions of care to gather essential patient information.
- Assist in coordinating referrals, education, and community resources.
- Actively participate in initiatives aimed at enhancing patient experience and care quality.
- Engage in daily huddles with the provider.
- Serve as a patient experience advocate, maintaining professionalism and prioritizing customer service in all interactions.
- Handle patient information confidentially, in accordance with privacy policies.
- Perform various front office duties as necessary.
- May require some regional travel; a valid driver's license is necessary for travel.
- Participate in on-call responsibilities as required.
- Other duties as assigned.
MINIMUM REQUIRED QUALIFICATIONS:
- Completion of a LPN or RN educational program.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the state nursing board.
- AHA CPR/BLS certification required within 30 days of hire.
- This role necessitates occasional regional travel; ability to travel to multiple sites is required.
- One to two years of nursing experience in a primary or specialty care physician's office.
- Prior experience in triage, emergency room, cardiac care, intensive care, or related specialty nursing.
