Case Manager

EAC Network is a not-for-profit organization empowering individuals in need across Long Island and NYC. We're seeking a dedicated Case Manager to join our Substance Use Disorder (SUD) Program, providing life-changing support to those transitioning back into the community after incarceration.
Key Responsibilities:- Work directly with individuals preparing to re-enter society from Suffolk County correctional facilities, offering crucial support in substance use recovery and transition planning.
- Collaborate with correctional facility staff and community organizations to develop customized reentry plans that connect clients to post-release services, such as healthcare, housing, and vocational support.
- Facilitate access to essential services like Medicaid, benefits, and Medication Assisted Treatment (MAT) providers, ensuring smooth transitions and reducing the risk of relapse.
- Act as a trusted guide for individuals re-entering the community, providing ongoing care management and connecting them with peer support networks.
- Track client progress, maintain accurate records, and contribute to a data-driven approach to improve outcomes for those in need.
- A Bachelor's Degree in Social Work, Mental Health Counseling, Criminal Justice, or CASAC, with at least two years of experience working with substance use or criminal justice-involved populations.
- Passion for helping vulnerable individuals overcome challenges and succeed in their recovery journey.
- Strong communication and collaboration skills, with the ability to work with a wide network of community and healthcare services.
- Proficiency with digital tools and systems, including Microsoft Office and client databases.
- A valid NYS driver's license, personal vehicle, and insurance to help transport clients to appointments when needed.
