Key Responsibilities
  • Counseling and Support: Provide emotional support and guidance to patients and their families, helping them understand healthcare recommendations and options.
  • Needs Assessment and Referral: Conduct comprehensive assessments to identify patients' needs, and refer them to appropriate community resources, such as healthcare services, social services, and education programs.
  • Care Planning and Coordination: Work closely with care teams to develop care plans for identified patients, ensuring they receive the necessary support and resources.
  • Documentation and Charting: Accurately document patient visits and interactions in our electronic health record system.
  • Patient Education and Outreach: Provide education and materials to patients on general health and wellness, as well as specific health topics, such as chronic disease management and healthy lifestyle choices.
  • Community Outreach and Partnerships: Participate in community outreach activities, building relationships with local agencies and organizations that serve vulnerable populations, and identifying opportunities for collaboration and resource sharing.
  • Support for Mobile Medicine and Health Equity Initiatives: Provide resource navigation and front office support for Mobile Medicine efforts, and contribute to the development and implementation of Health Equity initiatives.
  • Supply Distribution and Education: Assist in the distribution of essential supplies, clothing, and hygiene items to those in need, and provide education on healthy habits and self-care.
  • Relationship Building and Student Support: Foster relationships with local agencies and organizations, and support student rotations and training activities.
Requirements
  • Education and Experience: One year of experience in health and/or social services, with a valid driver's license and insurance.
  • Skills and Qualifications: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. Computer skills and a commitment to supporting a safe, respectful, and inclusive environment are also essential.
Work Environment

The work environment for this role is dynamic and fast-paced, with a focus on providing high-quality patient care and community support. The successful candidate will be required to stand, sit, and be mobile for up to one-third of the time, with frequent reading and computer screen use. Lifting up to 10 pounds is common, with less frequent lifting of 25-40 pounds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this role.
