Community Health Advocate

We are seeking a highly motivated and compassionate Community Health Worker to join our team at CHAS Health. As a Community Health Worker, you will play a vital role in improving the overall health of the communities we serve by expanding access to quality health and wellness services for maternity and young pediatric populations.
Key Responsibilities- Counsel and support patients and their families to understand healthcare recommendations and options.
- Conduct intakes and assessments of client needs in accordance with First Steps Program guidelines, identifying comprehensive and ongoing needs such as healthcare, social, and education.
- Advocate and refer patients to appropriate community resources, working closely with care teams to develop care plans for identified patients.
- Teach maternal and infant care related to individual and community welfare, as required.
- Assist in special studies, research programs, and resource development, documenting progress and following up as necessary.
- Bachelor's degree in a social service-related field, such as social work, behavioral sciences, psychology, child development, or mental health.
- Associates degree in social service-related field required, with a minimum of two years of health and/or social services experience.
- Excellent oral and written communication skills, with compassion for patients and ability to work under tight time constraints, handle high stress, and changing priorities.
- Self-motivated with ability to work independently and as part of a team, with a valid driver's license and insurance, and BLS (CPR/AED) certification.
Required to stand, sit, and be mobile one-third to two-thirds of the time, with frequent use of hands to finger, handle, or feel, and occasional climbing or balancing, stooping, kneeling, or crouching.
Communicating by talking/hearing occurs over two-thirds of the day, with occasional lifting or exerting force up to 10 pounds, and less than one-third of the time up to 25 pounds.
