We are seeking a highly skilled Patient Care Coordinator to join our team in the Medical Surgical Unit. This is an exciting opportunity for a dedicated and compassionate healthcare professional to make a meaningful difference in the lives of our patients.

About the Role:

This is a full-time position that requires collaboration with cross-functional teams, including physicians, nurses, and other healthcare professionals. The Patient Care Coordinator will be responsible for providing high-quality patient care, administering treatments, and documenting patient progress.

Responsibilities:

  • Coordinate patient care across varying levels of acuity, from post-operative to critical care cases, including pediatric patients.
  • Collaborate daily with healthcare teams to achieve optimal patient care outcomes.
  • Administer intravenous therapy, perform complex wound care, and monitor drainage devices.
  • Supervise patient care assistants to ensure high standards of patient care.
  • Mentor and coach less experienced staff members to foster a collaborative environment.
  • Document patient care plans, including nursing diagnoses, expected outcomes, and selected interventions.
  • Assess for physiological and psychosocial changes in patient conditions.

Qualifications:

  • Graduate from an accredited school of nursing.
  • Active RN License.
  • Basic Life Support (BLS) Certification.
  • Minimum of 2 years of experience in a medical-surgical setting.

Preferred Qualifications:

  • Strong leadership and mentorship skills.
  • Experience with pediatric patient care in surgical and critical care settings.

Certifications:

  • RN License
  • BLS Certification

Salary Range: $80,000 - $110,000 per annum, depending on experience.
