As a vital member of our healthcare team, you will be responsible for managing the intake procedures for new patients and determining suitable candidates for home care services. Your role involves evaluating patient referrals to assess their eligibility for admission.
Key Responsibilities
- Collect comprehensive patient information, including demographics, insurance details, and medical history to complete the referral process.
- Verify initial and ongoing insurance/managed care benefits with relevant providers.
- Facilitate timely admissions by ensuring the prompt acquisition of medical records.
- Conduct welcome calls to patients or their representatives post-intake, once a clinician has been assigned.
- Address any inquiries from patients and their representatives.
- Maintain and update databases and spreadsheets with current data and the status of admission referrals.
- Daily scanning of patient documentation into the electronic medical record (EMR) and ensuring accurate patient charting.
- Provide the care team with reports on pending admissions, new admissions, and outstanding items.
- Collaborate with colleagues and management to effectively resolve issues affecting the admission process.
- Escalate any admission denials to leadership for further consideration.
- Adhere to all local, state, and federal regulations.
- Assist in implementing improved workflows and procedures to ensure compliance with admission policies.
- Foster positive relationships with current and potential referral sources.
- High school diploma, General Equivalency Diploma (GED), or higher required upon hire.
- A minimum of 2 years of relevant experience is required.
- Current licensure or certification in a clinical specialty is required upon hire.
- Valid driver's license and a minimum age of 21 required upon hire.
- Demonstrates critical thinking skills and decisive judgment.
- Ability to work independently with minimal supervision.
