Job Description

We are seeking a skilled Nurse Clinician in Mental Health and Substance Abuse to join our team at Cabarrus Rowan Community Health Centers. Key Responsibilities
  • Develop and implement individualized treatment plans for patients with complex mental health and substance use conditions.
  • Coordinate care for patients with complex mental health and substance use conditions, ensuring seamless integration throughout the multidisciplinary team.
  • Monitor and facilitate patient referrals, follow-up appointments, and transitions among internal and external care providers and agencies.
