Nurse Clinician in Mental Health and Substance Abuse

We are seeking a skilled Nurse Clinician in Mental Health and Substance Abuse to join our team at Cabarrus Rowan Community Health Centers. This is a full-time position that requires the candidate to have a strong background in nursing, with a focus on mental health and substance abuse. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities- Develop and implement individualized treatment plans for patients with complex mental health and substance use conditions.
- Coordinate care for patients with complex mental health and substance use conditions, ensuring seamless integration throughout the multidisciplinary team.
- Monitor and facilitate patient referrals, follow-up appointments, and transitions among internal and external care providers and agencies.
