LPN Community Care Specialist

We are seeking a skilled Licensed Vocational Nurse to join our team at ClearSky Health. As a LPN Community Care Specialist, you will provide high-quality care to patients, under the direction of a Registered Nurse and in accordance with physician's orders and established standards and protocols.
Key Responsibilities- Provide skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
- Implement orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the Registered Nurse.
- Provide information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
- Report to supervisor on patient care rendered, problems concerning patients' needs, and resolution to those needs.
- Two years' experience in inpatient hospital setting, inpatient rehabilitation experience preferred.
- Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
- Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
- Demonstrates general computer skills including data entry, word processing, email, and records management.
- Visual acuity required for patient assessment and documentation of care.
- Sufficient manual dexterity to operate equipment and computer keyboard.
