Primary Care Physician Opportunity

We are seeking a dedicated and passionate Primary Care Physician to join our outpatient clinic in Louisville, KY. Our clinic is committed to providing collaborative, value-based care to our patients.

Responsibilities:
  • Conduct comprehensive initial assessments and ongoing evaluations of patients' health status.
  • Develop personalized treatment plans tailored to meet patients' healthcare needs, incorporating evidence-based medicine and best practices.
  • Provide primary care services, including routine physical exams, preventive care, chronic disease management, and acute illness management.
  • Collaborate effectively with other healthcare providers to ensure coordinated and holistic patient care.
  • Educate patients on disease prevention, health promotion, and management of chronic conditions.
  • Utilize electronic health records for documentation, ordering tests, prescribing medications, and communicating with patients and other members of the healthcare team.
  • Participate in quality improvement initiatives and practice transformation activities to enhance patient outcomes and promote value-based care delivery.
Qualifications:
  • Medical degree from an accredited medical school and completion of residency training in Family Medicine or Internal Medicine-Pediatrics.
  • Board certification or eligibility for certification in Family Medicine or Internal Medicine-Pediatrics.
  • Current, unrestricted license to practice medicine in the state of Louisiana.
  • DEA registration for prescribing controlled substances.
  • Demonstrated commitment to providing patient-centered, evidence-based care in a collaborative team environment.
