Primary Care Physician

We are seeking a highly skilled and experienced Primary Care Physician to join our team as a Clinical Lead Physician. In this role, you will be responsible for providing high-quality medical care to residents and patients across various settings, including skilled nursing facilities, assisted living, and private homes.
Key Responsibilities- Provide complex and geriatric care to residents and patients in partnership with non-physician practitioners and other interprofessional team members.
- Collaborate with facility leadership teams, nurse practitioners/physician assistants, patients, and families to ensure high-quality care.
- Assume collaborating physician duties for nurse practitioners and physician assistants, providing support and guidance in their clinical roles.
- Audit medical orders and documentation for each provider monthly, ensuring compliance with state and regional regulatory frameworks.
- Participate in company frontline and backup call processes for timely response to acute changes of condition.
- Focus on quality outcomes, including care path compliance, productivity, prevention of avoidable transfers, and decrease in polypharmacy and costs of care.
- Graduate from an accredited school of medicine and a Doctor of Medicine degree.
- Two or more years of administrative leadership experience in healthcare.
- Clinical medical experience of seven years or more preferred.
- Board certification in Family Medicine, Internal Medicine, or Geriatrics; qualification in Geriatrics preferred.
- Practicing physician currently licensed under the provisions of the State Board of Medicine.
- Active unrestricted licensure to practice in the state of employment.
- Current Drug Enforcement Administration registration.
