Job Title: Oncology Navigator

Job Summary:

We are seeking an experienced Oncology Navigator to join our team at CommonSpirit Health. The successful candidate will be responsible for providing seamless navigation of patients through the continuum of care, from diagnosis to survivorship. This role requires strong communication and organizational skills, as well as the ability to work collaboratively with healthcare teams to ensure timely access to care.

Key Responsibilities:

  • Facilitate patient care services to ensure excellence in patient care and patient flow.
  • Assist with timely access to care by coordinating/facilitating appointments.
  • Accompany patients to consultation appointments.
  • Follow patients from diagnosis through survivorship.
  • Provide seamless referrals to resources to ensure elimination of barriers to care.
  • Assess patient's current and future needs, provide patient access to resources (internal and external), facilitate distress screenings and patient needs assessments and make appropriate referrals as needed.
  • Act as a patient's advocate.
  • Facilitate the development, implementation and adherence to clinical and research guidelines, as appropriate, to ensure optimal outcomes.
  • Work with research teams and physicians to make sure patients are appropriately screened for possible participation in research trials.
  • Establish and maintain close collaboration with physicians and the cancer care team to ensure seamless patient care and care coordination.
  • Actively participate in a Multi-disciplinary Conference.
  • Provide patient education and symptom management support.
  • Tracks navigation program metrics and quality indicators; documents patient interactions and progression and communicates navigation activities to physicians and the cancer team.
  • Documents all patient care and telephone communications.
  • Collaborates with director to plan, develop, implement, market and evaluate new or expanded programs and services.
  • Participates in community health promotion and awareness programs.
  • Outreach: Conduct breast health education classes with individuals and aggregates.

Qualifications:

  • Required Education: Associates Degree, Nursing
  • Required Experience: Five years nursing experience
  • Required Licensure and Certifications: Registered Nurse License from the Texas Board of Nursing
  • Required Minimum Knowledge, Skills, Abilities and Training: Organizational and Planning techniques. Effective Written and Verbal Communication. Time Management. Computer skills in database management and forms development.
