Hospice RN Case Manager Job Description

Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a Hospice RN Case Manager, you will provide exceptional care to patients and families, working collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care.

Responsibilities:

  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct comprehensive assessments of patients and families, identifying their unique needs and developing personalized care plans.
  • Participate in on-call duties, providing 24/7 support to patients and families.
  • Collaborate with the Interdisciplinary Group (IDG) to develop and implement care plans, ensuring seamless care coordination.
  • Evaluate and coordinate the need for services, ensuring patients receive the care they need to achieve their goals.
  • Supervise and direct the services provided by LPNs, Hospice Aides, and Homemakers, ensuring high-quality care delivery.
  • Facilitate the consideration of alternative levels of care, such as inpatient, respite, or continuous care, when indicated by patient needs.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years, or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with preference for precepting or training experience.
  • CPR certification.
  • Able to provide proof of valid driver's license and auto liability insurance.
