Job Summary:

We are seeking a skilled Case Manager RN to join our team at Memorial Healthcare System. As a Case Manager RN, you will play a critical role in ensuring the seamless transition of patients from hospital to home or other healthcare settings.

Key Responsibilities:

  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Coordinate discharge planning, including arranging for post-acute care and community resources
  • Provide education and support to patients and families to promote self-management and independence
  • Communicate effectively with healthcare providers, payers, and other stakeholders to ensure timely and efficient care transitions
  • Monitor and evaluate patient outcomes to identify areas for improvement

Requirements:

  • Current RN licensure in the State of Florida
  • Minimum one year of experience in hospital-based case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
