Job Summary:

We are seeking a skilled Case Manager RN to join our team at Memorial Healthcare System. As a Case Manager RN, you will play a critical role in ensuring the highest quality patient care and outcomes.

Key Responsibilities:

  • Collaborate with patients, families, and healthcare teams to develop and implement effective care plans.
  • Conduct thorough assessments of patient needs and develop strategies to address complex care requirements.
  • Facilitate communication between patients, families, and healthcare providers to ensure seamless transitions of care.
  • Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies to improve outcomes.

Requirements:

  • Current RN licensure in the State of Florida.
  • Minimum one year of experience in hospital-based Case Management practice or five years in Healthcare.
  • Basic Life Support (BLS) Healthcare Provider certification.

Working Conditions:

This role requires critical thinking skills, effective communication, decisive judgment, and the ability to build and foster positive relationships. The incumbent must be able to lead others and take appropriate action when required.

Shift:

Primarily for office workers - not eligible for shift differential.
