Job Summary

Key Responsibilities
  • Coordinate patient care services, ensuring seamless transitions and efficient use of resources.
  • Develop and implement comprehensive care plans, collaborating with healthcare teams to achieve optimal patient outcomes.
  • Conduct thorough assessments, identify patient needs, and provide education and support to promote health and wellness.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure quality patient care and effective communication.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
Requirements
  • Bachelor's degree in Nursing or related field.
  • Nationally accredited nursing graduate.
  • Current licensure as a Registered Nurse in the state of New Mexico.
  • Basic Life Support (BLS) certification.
  • Ability to communicate effectively with patients, families, and healthcare teams.
  • Strong problem-solving and critical thinking skills.
Preferred Qualifications
  • Bilingual English/Spanish or other languages spoken in New Mexico.
  • Experience working in a hospital or healthcare setting.
  • Knowledge of electronic medical records and healthcare software.
