Key Responsibilities
As the RN Case Manager, you will take full nursing responsibility for implementing the Plan of Care for each hospice patient, consistently assessing both personal and professional performance to enhance productivity and the quality of care provided.
- Conducts the initial nursing assessment to determine eligibility for hospice services within forty-eight (48) hours of referral.
- Evaluates the physical, psychosocial, and environmental needs of the patient and family, reassessing at least every fifteen (15) days.
- Initiates and coordinates the care plan while maintaining precise and relevant clinical documentation regarding the patient's status.
- Records issues, appropriate objectives, interventions, and responses from the patient and family regarding hospice care.
- Collaborates daily with the patient/family, attending physician, and other interdisciplinary team members to provide comprehensive care.
Experience Requirements
- A minimum of one year of clinical experience.
- Current RN licensure in the state of practice along with one year of clinical experience.
- Valid CPR Certification.
- Current driver's license, valid vehicle insurance, and access to reliable transportation or public transit.
- ID: No additional state-specific requirements.
