As a Registered Nurse Care Coordinator, your responsibilities will include conducting in-home admissions, performing supervisory visits, managing office tasks, and carrying out semi-annual or annual assessments of field caregivers.
Our team is committed to ensuring compliance with client assessments and service plan standards that align with state regulations.
Work Schedule: Typically Monday - Friday, 9:00 AM - 5:00 PM, with flexibility to meet business needs.

Key Responsibilities:
- Conduct initial evaluations and regularly reassess clients' nursing needs.
- Provide counseling to clients and their families regarding nursing and related needs.
- Coordinate and develop service plans for clients based on assessments and admissions.
- Supervise field staff to ensure effective implementation of new Medicaid Waiver programs.
- Guarantee quality care for clients by creating, implementing, monitoring, and enforcing quality assurance policies and procedures.
- Assist branch teams with pre-service training and orientation, as well as in-service training for caregivers.
- Support with additional duties as assigned by the Branch Manager.
Qualifications:
To be considered for this role, candidates should possess:
- A current RN license in the state of service.
- A valid driver's license.
- Access to reliable and insured transportation.
- The ability to travel within surrounding counties 2-3 days per week during standard hours.
- Availability to be On-Call for caseload-related emergencies.
- Preferred nursing experience.
- Compliance with state background screening requirements.
