LPN Evening Shift at The Manor Health and Rehabilitation Center

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at The Manor Health and Rehabilitation Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.

About the Role

The LPN will be responsible for providing skilled nursing care to our patients, including administering medications, performing various medical procedures, and documenting patient information. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living.
  • Conduct thorough assessments of patients to identify their physical, emotional, and social needs.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Collaborate with the healthcare team to ensure seamless communication and coordination of care.
  • Document patient information accurately and thoroughly, including progress notes, medication administration records, and other relevant data.
Requirements
  • Graduate of an approved LPN program and licensed to practice in NJ.
  • Previous LPN experience preferred.
  • BLS certification required.
  • Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
