LPN Evening Position at The Manor Health and Rehabilitation Center

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our evening team at The Manor Health and Rehabilitation Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.

About the Role

The LPN will be responsible for providing skilled nursing care to our residents, including administering medications, performing vital signs, and assisting with activities of daily living. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.

Responsibilities
  • Provide skilled nursing care to residents, including administering medications and performing vital signs
  • Assist residents with activities of daily living, such as bathing, dressing, and grooming
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Monitor and report changes in resident condition to the RN and other members of the healthcare team
  • Participate in the development and implementation of the care plan, including the assessment, planning, implementation, and evaluation of resident care
Requirements
