Key Responsibilities:
- Establish and maintain positive relationships with patients and their families, providing education on self-care and self-administration of infusion therapy.
- Assess patient appropriateness for home infusion therapy, ensuring a safe and supportive environment.
- Obtain and document patient medical history, including medication profiles and treatment plans.
- Complete all required clinical documentation, including nursing plans of care and visit notes.
- Participate in case conferences to comprehensively plan patient care with the multidisciplinary care team.
- Administer medications as prescribed, performing therapy-specific clinical assessments and monitoring patient response.
- Communicate observations and patient response to therapy with the care team, including physicians, pharmacists, and registered dietitians.
Requirements:
- Active and unrestricted licensure as a Registered Nurse (RN) in the state(s) of practice.
- Current Basic Life Support (BLS) and/or CPR certification.
- 1+ years of experience working as an RN in a clinical care setting, contributing to proficiency in infusion therapy.
- Proficient with electronic medical record documentation.
- Ability to work independently in home or alternate-site settings, with a flexible work schedule.
Preferred Qualifications:
- Experience administering infusion therapy in a home or alternate-site setting.
- Knowledge of and experience with Microsoft Office and electronic medical records (EMR) programs.
- Knowledge of the Infusion Nursing Society (INS) standards of practice, policies, and procedures.
