Job SummaryThe Licensed Practical Nurse provides age-appropriate, population-specific patient care in a professional, compassionate, and culturally sensitive manner. You will work collaboratively with our healthcare team to deliver exceptional patient care and promote a positive patient experience.
Responsibilities- Serve as a mentor and resource for nursing standards, competencies, policies, and procedures.
- Collaborate with staff and other departments to develop strategies for continuous quality improvement and patient satisfaction.
- Utilize data to meet care coordination needs across settings, working with patients, families, and other care providers.
- Provide direct and indirect care that is safe, ethical, patient-centered, and evidence-based.
- Protect patients and employees by adhering to infection control policies and protocols.
- Prepare patients for encounters by following established protocols.
- Demonstrate competence in clinical skills and proper medication and vaccine management processes.
- Reconcile medication lists and prepare prescription renewals.
- Demonstrate complete, accurate, and timely documentation in the electronic medical record.
- Respond to patient phone calls and engage providers or RNs appropriately.
- Reinforce patient and family education plans that meet patient needs.
- Perform ordered point-of-care testing, including specimen collection and proper documentation.
- Assist with patient encounter flow and troubleshoot issues in collaboration with supervisors.
- Provide guidance and mentorship to new and incumbent staff based on skill sets.
- Attend meetings and serve on various organization committees.
- Assist supervisors with compliance and staff development duties.
- Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
- Complete health center daily, weekly, or monthly tasks as assigned.
- Ensure operation of medical equipment by following manufacturer instructions, troubleshooting malfunctions, and requesting repairs.
- Unrestricted current LPN license to practice nursing in New York State.
- Certificate from an accredited school of nursing.
- Minimum of 1 year of clinical experience preferred.
- Ability to travel throughout the Network.
- Current BLS certification.
- Proficiency with Microsoft applications and electronic medical records.
- Excellent verbal and written communication skills.
- Self-directed, detail-oriented, motivated, and ability to work independently and in a team atmosphere.
The pay rate for this position is $28.00/hour.
