About the Role

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team in Kings, NY, providing annual clinical assessments to patients in their homes.

This role is an exciting opportunity to be part of a seamless health journey for patients across the care continuum, addressing physical, mental, and social needs of patients wherever they may be.

The ideal candidate will have the ability to work independently and as part of a team, with excellent communication and interpersonal skills.

Key Responsibilities
  1. Conduct in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  2. Perform evidence-based practice screenings, including point of care testing (as appropriate).
  3. Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
  4. Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
  5. Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care.
  6. Identify urgent and emergent situations and intervene appropriately.
  7. Educate members on topics such as disease process, medication, and compliance.
