Compassionate Case Manager for Homeless Population

This role offers a unique opportunity to make a meaningful impact in the lives of individuals experiencing homelessness. As a Compassionate Case Manager, you will be part of a multidisciplinary team working together to provide comprehensive services and support to this vulnerable population.
About the Role
Under the supervision of the Program Manager and as part of a dynamic team, you will perform professional case management services, including outreach, linkage, and assessment of clients' immediate needs. You will work closely with consumers, their families, and other stakeholders to develop individualized treatment plans that address their specific challenges and goals.
Key Responsibilities:
- Conduct thorough assessments of clients' needs, developing personalized treatment plans to address mental, emotional, medical, and substance-related issues.
- Collaborate with internal and external entities, presenting information within a Multi-Disciplinary Team (MDT) setting.
- Provide peer support and life skills techniques to clients, facilitating group sessions as needed.
- Document client interactions, treatment plans, and outcomes in accordance with established standards of care.
- Maintain accurate records, ensuring compliance with HIPAA and CFR42 regulations.
Requirements
To succeed in this role, you must possess the following qualities and qualifications:
- A high school diploma or equivalent.
- Lived experience with homelessness and/or relevant training in Peer Counseling.
- Experience working with diverse populations, including those with co-occurring mental health and substance use disorders.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Proficiency in Microsoft Office Suite and Internet applications.
- Ability to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities.
- Familiarity with HMIS systems and documentation guidelines.
- Willingness to work evenings, weekends, and holidays as needed.
Salary and Benefits
The salary range for this position is $21-$23 per hour.
