We are seeking a skilled Patient Navigator to join our team.

About the Role
This role will support patient navigation efforts by providing telephonic and in-person support to patients who face barriers accessing healthcare. You will serve as a liaison between individuals, communities, and health and social services to facilitate access to care, improve service delivery, and address social determinants of health. This position requires strong communication and interpersonal skills, with the ability to travel throughout the assigned region and conduct home visits.

Key Responsibilities
* Provide telephonic and in-person support to patients facing barriers accessing healthcare
* Serve as a liaison between individuals, communities, and health and social services
* Facilitate access to care and improve service delivery
* Address social determinants of health

Requirements
* High school degree or equivalent
* Ability to obtain Community Health Worker Certification within first year of employment (if not currently certified)
* 1+ years of experience in a physician's office or clinical setting
* Proficient computer skills to work efficiently with electronic medical records
