This role involves assisting patients and their families in navigating and accessing community services, resources, and promoting healthy behaviors through health promotion, disease prevention, and management.

Key Responsibilities:
  • Provide social support and goal setting, health behavior changes, and advocacy for individuals and community health needs.
  • Schedule, reschedule, and cancel appointments, including phone call screening and transferring, answering patient questions, appointment confirmations, and patient registration.
  • Offer exceptional customer service while working in a busy medical clinic environment.

Requirements:
  • High school diploma or GED equivalent.
  • Minimum of two years' experience, with at least one year in a related field.

Salary: $16.00 to $21.53 hourly, commensurate with education and experience. Benefits include 401(k) retirement, 7 paid holidays, medical, dental, vision insurance, and more.
