As a Registered Nurse (RN) on our Home Health team, you'll play a vital role in providing high-quality care to patients and their families. Responsibilities
As a Home Health RN Case Manager, your responsibilities will include:
- Performing patient assessments and collaborating with the care team to develop and implement a plan of care.
- Making referrals to other disciplines as indicated by the patient's identified needs or documenting rationale for not doing so.
- Promoting patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
- Supervising LPNs and HHAs.
- Completing documentation timely and accurately.
- Regularly communicating patient progress to the clinical manager and care team.
- Planning and providing staff education.
- Performing on-call responsibilities and on-call services to patients/families as assigned.
- Participating in clinical development and continuing education programs.
Qualifications
To be successful in this role, you'll need:
- One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
