Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Home Care. As a key member of our care team, you will be responsible for performing patient assessments, collaborating with the care team to develop and implement a plan of care, and promoting patient health and independence through teaching and rehabilitative measures.
Responsibilities- Perform patient assessments and develop a plan of care in collaboration with the care team.
- Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
- Promote patient health and independence through teaching and rehabilitative measures.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide staff education.

RequirementsTo be eligible for this role, you must have a current RN license and at least 6 months of clinical experience as a RN. If you have less than 6 months of clinical experience, you must participate in our RN Intern program.
