Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and national standards, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans, focusing on patient-centered care and promoting independence.
- Maintain accurate and detailed records of patient interactions, progress, and outcomes.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and patient satisfaction.
- Participate in ongoing education and training to enhance clinical skills and stay up-to-date with industry developments.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
