Job Summary:

We are seeking a skilled Case Manager to join our team. The successful candidate will provide individual and group mental health and substance use disorder case management services, referring and advocating for additional services as needed.

Key Responsibilities:
  • Provide individual and group mental health and substance use disorder case management services
  • Refer and advocate for additional services as identified
  • Develop and implement treatment plans with input from clients and families
  • Conduct clinical behavioral health assessments
  • Enter written documentation into Electronic Medical Record in a timely manner
  • Liaise with local law enforcement and stakeholders to maintain coordination of the treatment process
  • Complete all required agency trainings to maintain 100% compliance
  • Maintain departmental productivity expectations
Work Environment/Physical Demands:

This position operates in community-based, office-based, and/or inpatient-based settings. The successful candidate will be required to sit, stand, bend, lift, drive, and move intermittently during work hours.

Position Type, Expected Hours of Work and Travel:

This is a full-time (40 hours/week) position with a flexible work schedule to include early morning and evenings. On-call rotation and weekend hours may be required.
