Role Overview

As a Geriatric Care Provider with ChenMed, you will be responsible for delivering high-quality patient care in our medical centers. This role requires a strong commitment to preventive care and a passion for improving health outcomes for seniors.

The estimated salary for this position is $120,000 - $150,000 per year, depending on experience.

Key Responsibilities
  • Patient Assessment and Management: Conduct comprehensive physical assessments and develop personalized care plans to address the unique needs of each patient.
  • Collaboration and Communication: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Health Education and Counseling: Provide patients and their families with education and support to promote healthy behaviors and disease management.
  • Documentation and Record-Keeping: Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
Requirements and Qualifications
  • Education: Master's degree in Nursing or related field.
  • Certification: Current certification as an Advanced Practice Registered Nurse (APRN) or similar advanced degree in Nursing required.
  • Experience: Minimum 1 year of clinical experience preferred, but willing to consider strong new graduates.
  • Skills and Abilities: Strong critical thinking and problem-solving skills, excellent communication and interpersonal skills, and ability to work well under pressure.
