Key Responsibilities:
  • Complete MDS assessments with a focus on accuracy, reimbursement maximization, and regulatory compliance
  • Oversee, coordinate, and coach team members delivering nursing care to residents
  • Collaborate with the interdisciplinary team to develop and implement person-centered care plans
  • Accurately observe and monitor residents' condition on an ongoing basis
  • Delegate data collection within scope of practice and respond to findings in a timely manner
  • Coordinate care and services with other departments/providers from admission to discharge
  • Ensure adherence to policies and procedures, State and Federal requirements, rules, and regulations as they relate to nursing care in the Skilled Nursing Facility
Requirements:
  • Graduate from an accredited Registered Nursing program
  • 1+ year(s) licensed nurse experience in a Skilled Nursing Facility (SNF)
  • Experience in MDS assessments and care planning
  • Ability to travel to any Ecumen facilities; travel may require overnight stays
  • Ability to follow all safety rules, regulations, policies, and procedures of the facility
